Managing Continuity of Care for Children with Special Needs in Rural and Remote Parts of Northern Ontario

Developed jointly by CRaNHR, Integrated Services for Northern Children (ISNC), and the Centre of Excellence for Children and Adolescents with Special Needs, this project compared alternative models - either using paraprofessional providers or volunteer mediators - for managing continuity in the care of children with special physical, mental health, and education needs in the non-urban parts of northern Ontario. Data collection involved 330 chart reviews and 100 interviews with care providers and parents/guardians. The analysis focused on identifying factors facilitating or impeding continuity of care.
